AccordantCare: How we help

Learn how the AccordantCare program provides one-on-one nurse support for members with rare conditions, on your terms.

It's never been more important to keep up with your health needs,  especially if you have a complex condition.  We're Accordant, and we're here to help with a support program for people living with conditions like yours.  

For more than 20 years, we've been helping people just like you make managing their complex conditions a little easier.  Here's how we do it. With one-on-one support from a dedicated nurse who specializes in your condition. Imagine being able to contact a nurse familiar with your needs by phone or secure message to help you understand symptoms, side effects, or flare ups when your doctor isn't available.  Or getting emotional support to help ease your mind when you have a question.  And a care team available 24-7  to help you understand your health plan and benefits, making sure you can get the medication, supplies, financial support, and other support resources you need to manage your health. 

Your nurse and care team are able to access real-time information about doctor visits, medication changes, or test results. They can help you find ways to stay on track, prevent complications, and make informed health decisions.  With our secure online portal, MyChart, you or your caregiver can see or manage your treatment plan, find trusted information about your condition, and securely message your care team or nurse anytime.  

The best part? It's available to you at no extra cost and on your terms. which means you decide how much extra support you need, how and when you need it.  Living with a complex condition can be difficult, but with extra help from a cordon, you can worry less about managing your condition and instead focus on living your best life.
