The great thing about accordant is that they do a wonderful job of managing the whole member, so they're not just managing for the MS, the rheumatoid arthritis. They're also helping manage the diabetes, the hypertension, the heart disease.

With all that being said, that can't be done unless you do have a good relationship.

Without Accordant, we might be able to address some of their needs, but we're not going to address the specific ones. And where that's going to cost us is that they're going to have problematic care in the future. Which is going to mean hospital stays. And we were talking about the 19 diseases that accordant represents. Those are some very costly medications.

And what we've done as a plan working alongside of accordant, and accordant has been a great partner, is basically bringing all of our vendor partners together. Because you have those relationships in place, that's what helps it work so well.

Dr. Rogen:

The Cleveland Clinic has vast experience, but for these really rare conditions where even the clinic may only see four, five, or six patients a year and our health plan in particular may only have one or two, we can't replicate what Accordant is able to do for us, where they may have 300 or 400 or 500 of these patients across the country that they're working with. So I wouldn't say that they have more clinical expertise than the clinic can bring to bear, but they have a lot more expertise in managing these patients long term over time from a socioeconomic point of view, from a telephonic care coordination point of view, from a coaching and health engagement point of view, and that's the big advantage that we found once we started working with Accordant.

Patients like the ability to not be sitting on the phone for 45 minutes waiting for someone to pick up. They like the ability of not having to leave a message and basically be told someone will call you back tomorrow with that information. They like the ability of getting someone that they know on the line that they trust that they've worked with, in some cases, for years and years and years, and ask the questions and get the answers they need and feel reassured. And again I can't emphasize enough the importance of the coaching and the education. This is not just, "hi, Mrs. Smith. How are you today? Did you take your pills? Wonderful. I'll call you in a month." it's beyond that, and that's where the real value of the program comes in.
